PReS-FINAL-2167: What is the potential of intra-articular corticosteroid injections to induce sustained remission in children with juvenile idiopathic arthritis?

چکیده
Introduction Intra-articular corticosteroid injections (iacis) are widely used in the management of children with juvenile idiopathic arthritis (JIA). Although this therapeutic intervention is generally considered for the treatment of children with arthritis in a small number of joints, the strategy of injecting simultaneously multiple joints has been advocated in children with polyarthritis. However, the relative indications of single and multiple iacis in the management of JIA remains unclear and controversial. Furthermore, the capacity of iacis to induce sustained remission of synovitis in injected joints is still poorly documented.

Introduction Over the last 3 decades there have been important advances in the management of juvenile idiopathic arthritis (JIA), which include the introduction of methotrexate and, later on, the tendency toward its earlier initiation, the widespread use of intra-articular corticosteroid injections, and, more recently, the availability of the biologic response modifiers. Although this therapeut...

Introduction Intra-articular corticosteroid injection (IASI), a common procedure in juvenile idiopathic arthritis (JIA), is usually associated with anxiety and pain. In previous study we concluded that nitrous oxide (NO) provides effective and safe sedation for such procedures. The efficacy in reducing pain was associated with the level of the child’s anxiety even before starting the procedure....
متن کاملGlucocorticoids in paediatric rheumatology.
Glucocorticoid (GC) drugs are the most potent anti-inflammatory agents available for the treatment of paediatric rheumatic diseases. These medications are used for the management of extra-articular features of systemic juvenile idiopathic arthritis and are the mainstay of therapy in children with juvenile systemic lupus erythematosus, juvenile dermatomyositis, and systemic vasculitis. The gener...
متن کاملA reappraisal of intra-articular corticosteroid therapy in juvenile idiopathic arthritis.
Intraarticular corticosteroid (IAC) injection is a safe and rapidly effective treatment for synovitis in children with juvenile idiopathic arthritis (JIA). This procedure can be performed in an ambulatory care setting using local anaesthesia, with or without conscious sedation. Younger children, or those candidate to multiple injections, require general anaesthesia. Triamcinolone hexacetonide i...
متن کاملIntra-articular corticosteroid injections in juvenile idiopathic arthritis
Therapeutic intervention with intra-articular steroid injections in juvenile idiopathic arthritis (JIA) has evolved from experience with adults with inflammatory joint disease, with the earliest report being published in 1951. The technique has subsequently been introduced into paediatric rheumatology practice, although much of the evidence supporting its use remains anecdotal or based on open,...
